Voting rights template

Hi Guys

I have a client who is trying to source a card device from a bank. The bank requires confirmation of "voting rights" on a letterhead. Where do I get this voting right template?

They mean confirmation of the shareholding.

They speak a different language to us mere mortals, and frequently have no clue what they themselves mean. But it's on the checklist.
