    Notification of danger Annexure form

    Good Morning All
I have tested a residential installation and got 376V supply coming to the DB instead of the normal 220V.I need to submit the notification to the local municipality. Kindly assist with the notification Annexure form if you have any.
    Kind regards

There is a notification form in SANS 10142-1 Annexure H (Notification of a Potential danger)

If you have 376V on a single phase supply then the utility has a floating neutral and you need to switch off the supply to the DB and log a call directly on the faults number for the area .
If you leave it , everything will blow or has already blown with overvoltage
